WHAT IS INCLUDED - 9 pages, 5 sheets
- ABC Data Sheet - 8 tall rows: date and time, setting, antecedent (what happened right before), behavior in observable words, consequence (what happened right after), length or intensity, and codes, with a note on observable words and a box for patterns noticed this week
- Frequency Count Data Sheet - for goals written as "will decrease to __ per __" or "no more than __ times": 13 dated rows by 8 periods, total, rate per unit of time and initials, with a note on why the periods have to stay the same length
- Interval Recording Sheet - for behaviors too frequent or too continuous to count: 13 observations by 10 intervals, plus count and percent, with partial, whole and momentary defined so a sheet never mixes them
- Scatterplot - 15 half-hour time blocks (8:00 to 3:00, editable) by two weeks of school days, shaded 1 to 3 by how much happened, so the dark cells show the time of day or the day to look at next
- Weekly Behavior Summary - 6 behavior rows by five days, week total, rate or percent, trend, and the decision (keep, adjust, fade, met), with the four decision rules on the page and a box for what changed this week
- How to Use These Five Pages - a table matching the goal's or the plan's wording to the sheet, the five-step routine, the seven antecedent codes and seven consequence codes the ABC sheet uses, the marks, and the Friday habit

What This Is Not
Not a behavior plan, not a functional behavior assessment, and not a list of strategies. The antecedent and consequence codes are for recording what happened, not for deciding why; the hypothesized function, if your team has one, comes from the FBA or BIP. It is a print PDF: nothing here calculates the rate or the percent, and it is not a Google Sheets file or a spreadsheet.
