Eight print-and-go data sheets that match how IEP goals are actually written, plus a one-page cheat sheet that tells you which sheet fits which goal.
What Is Included: 15 Pages, 8 Data Sheets
- Trial-by-Trial Data Sheet: ten trials per session, a correct count and a percent column, for goals written as "8 of 10 trials" or "80% accuracy"
- Frequency Count Sheet: tally, time in and time out, total and a per-hour rate, so counts from sessions of different lengths can be compared
- Duration & Latency Recording: how long a behavior lasts and how long it takes to start, on one sheet
- Interval Recording Sheet: ten intervals marked Y or N, with partial and whole interval explained
- Prompt Level Data Sheet: the least intrusive prompt needed, from Independent through Verbal, Gestural, Model and Partial Physical to Full Physical
- Weekly Progress Summary: Monday to Friday for each goal, a weekly average and a trend column

How To Use It
Print the sheet that matches the goal, one per student per goal, and fill it in by hand. On Friday, move the week's numbers to the summary page and plot the average on the graph.
What This Is Not
A print PDF to fill in by hand, not a fillable form or a spreadsheet: nothing calculates a percent automatically. It is not legal or clinical guidance; the student's IEP governs what is measured and reported.
