About this resource

Three math skills, three IEP progress monitoring packs, one download. Each pack is a complete kit for one skill: a goal page you edit, equivalent probe forms, an answer key, a graph and a data log. Baseline on Monday, goal written by Tuesday, first data point on Friday.

In Every Pack

Who Is It For

Special education case managers, resource room and self-contained teachers, interventionists and homeschool families who track math skills toward an IEP goal. Buy the three together when a caseload has students working on more than one of these skills.

Type In, Or Print

Each of the 3 files is a fillable PDF: click a line or a cell on the goal page, data log, caseload page or answer key, type, then save one copy per student. The probe forms print and the student writes by hand. It works offline with no account or subscription. It is not Google Slides, not Google Docs and not PowerPoint. Printing supplies are not included.

What This Is, And What It Is Not

A set of probes and the record-keeping around them. It is not a screener, a diagnostic test or a clinical tool, and it makes no claim about any student's eligibility, age or rate of progress. The sample goals are examples with brackets for your team to edit; the district's own rules and goal wording go on the blank lines. No claim of alignment to any standard.
