For Each Skill
- A goal page: what the probe measures, a sample goal written in the probe's own words (invented, every bracket edited by your team), the six-part goal frame, a baseline table for three probes and the median, benchmark rows, the administration and scoring directions, and lines for the student's own scoring rules
- Probe Form A: the page the student reads from or writes on
- A data log and graph: twelve rows, a y-axis already scaled to that probe's score, and the aim line drawn first
The Two Skills
- CVC Word Reading - 10 real words, two with each short vowel, in random order, so errors can be logged by vowel; score is words read correctly out of 10, untimed
- Addition Within 10 - 10 facts, vertical, no repeats within the form, sums from 1 to 10; score is problems correct out of 10, untimed

Both probes are untimed on purpose and every form is ten items, so a score of 8 means the same thing on either one and no student is scored on speed. The goal pages say so, so the condition stays the same all year.
The Full Pack
What This Is Not
Not a norm-referenced assessment. It carries no benchmark scores or "typical" rates; the criteria in the sample goals are in brackets because your team sets them from the baseline and the student's IEP. This free version has one form per skill; a weekly goal needs forms to rotate, which is what the full pack is for. It is a print PDF, not a Google Form or a spreadsheet, and it does not calculate or draw the graph.
