About this resource
A phonemic awareness record for use with the Heggerty program, or any phonemic awareness routine you teach: names down the side, a column for each skill, a page for each week. Not affiliated with or endorsed by Heggerty. This file has no Heggerty lessons, no lesson text and no word lists. You type your own item words and counts.
EVERYTHING INSIDE — 22 pages
- Class Record: 4 week pages, each with 16 student rows and a column for each of 8 skills (rhyme recognition, syllable blending, onset-rime blending, phoneme isolation, blending, segmenting, deletion and substitution). Print more copies for more weeks
- Class Summary Across Weeks: 16 student rows by 4 weeks, plus a box for the group you are thinking about
- My Item Words: type up to 7 item words for each of the 8 skills
- Student Profile: one child, 8 skills, Tried / Right / Missed boxes and a next step in your words
- Check Again: 4 checks across, a date row and one row for each of the 8 skills
- 2 one-on-one check sheets: one with 10 item rows and a Right ___ out of ___ box, one for taking a sound away and changing a sound with 9 rows
- My Sound Stars: a child page with 8 skills and 6 circles to color for each
- Word Cards to Cut: 9 cards in one block, each with a box for one of your words and 4 sound circles
- Sound Boxes to Cut: 6 strips with 2 to 5 boxes, one straight cut each
- A note home with blanks in your own words
- A phrase bank of 32 starters in 8 groups
- A filled-in example: one invented child, Dev, and invented practice items, every count re-added from its own table
- A binder cover, a spine label and 6 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
How to use it in three steps: (1) type your item words on page 5; (2) give your checks and type the number right in the class record each week; (3) print a profile for any child you want to look at closely, check again, and send a note home.
What This Is, And What It Is Not
It is a fillable PDF: type into it or print it and write by hand. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add up scores or copy numbers from one page to the next. You cannot drag the boxes around or restyle the pages.
There are no lessons, item lists, cutoffs, benchmarks or grade-level norms in it. The words are yours. What a score means, which children get which help and what happens next are your school's rules, and the next-step lines are left blank for your words. No health information is filled in. The example child is invented and says so on the page. The cutting is the 9 word cards, the 6 sound-box strips and the 6 binder tabs, each in one block with straight cuts.
