The five sheets a daily living, self-care or community goal is measured on, free, print and go: a task analysis data sheet with a prompt code in every cell, a daily living routine checklist for two routines across five days, a self-care skills inventory, a community outing data sheet, and a generalization check across three settings.

- Daily Living Routine Checklist - two routines a page (arrival, hygiene, a meal, clean-up), 7 steps each, Monday to Friday across, a prompt code per step per day, with steps-independent and initials rows for each routine
- Self-Care Skills Inventory - 16 skills you name, rated not yet, with help, or independent, with the date observed, the setting and a notes column, and a note on rating what was seen rather than what is expected
- Community Outing Data Sheet - 10 rows: destination, skill targeted, opportunities, independent, prompted, prompt level, and safety notes in observable words, plus a box for your district's outing rule and where the form lives
- Generalization Check - 12 skill rows, three settings across with an I or a prompt code in each, the dates, independent in all three, and who observed, with a box for the settings and adults available this quarter
- How to Use These Five Pages - a table matching the goal's wording to the sheet, three numbered steps, the six prompt codes every sheet uses (I, V, G, M, PP, FP), the marks, and the weekly habit

What This Is Not
Not a curriculum, not a list of routines, and not a prompting procedure. The steps of every routine are yours to write, and the prompt codes record the most help a step got; nothing here says which prompt to use, when to fade it, or what a student should be doing at any age. It is a print PDF: nothing here calculates the percent, and it is not a Google Sheets file or a spreadsheet.
