About this resource
Five early reading skills, each one start to finish: the IEP goal, the probe forms that measure it, and a graph scaled to the probe. One skill per file, so you print only the probe the goal names.
Every probe is untimed and ten items long, so a score of 8 means the same thing on every form of every skill, and no student is ever scored on speed.
WHAT IS IN THE BUNDLE - 5 fillable PDFs, 68 pages, 28 probe forms
- Letter Naming Progress Monitoring Probes - 13 pages, 5 forms. Ten upper- and lowercase letters a form, cut from one shuffle of all 52 so no letter appears on two forms
- Letter Sound Progress Monitoring Probes - 13 pages, 5 forms. All 26 lowercase letters across the set, 24 of them twice, so a sound the student cannot produce comes back inside the same rotation
- CVC Word Reading Progress Monitoring Probes - 14 pages, 6 forms. Two words with each short vowel on every form, so the error column names the vowel to reteach
- Blends and Digraphs Progress Monitoring Probes - 14 pages, 6 forms. One-syllable words with sh, ch, th, wh, ng or a beginning or ending blend, no word on two forms
- Sight Word Progress Monitoring Probes - 14 pages, 6 forms. Sixty of the Fry first hundred, none on two forms; a word that has to be sounded out is an error, because the goal is automatic recognition
What Every File Contains
- A goal page: what the probe measures, a sample goal (invented, every bracket edited by your team), the six-part goal frame as type-in fields, a baseline table for three probes and the median, benchmark rows, the administration and scoring directions, and lines for this student's own scoring rules
- The probe forms: ten items each, drawn the same way, rotated so the graph is the student and not the paper. Every form carries the goal it measures - the sample plus a type-in box for the goal as written for this student - so the goal prints on the sheet the person giving the probe is holding. A teacher scoring strip and a Correct ___ / 10 box sit at the foot of every form
- A data log and graph: twelve rows, a y-axis already scaled to the probe, the aim line drawn first
- A caseload summary: twelve student rows, baseline and criterion, eight probe columns, a trend column and a decision column. For the data meeting, instead of twelve folders
- A worked example on that skill: an invented student, twelve weekly probes, the baseline taken as a median, three points under the aim line forcing a teaching change, the criterion met, and the progress-report sentence the log produces
What This Is, And What It Is Not
Fillable PDFs. Goal pages, the goal box on every probe form, data logs and caseload pages are type-in fields: click, type, Save As once per student. The probe forms are meant to be printed; the student reads from paper and writes nothing, so one copy of each form in a sheet protector lasts the year. Works offline with no account or subscription. Does not calculate percents or draw the graph.
Not a norm-referenced assessment. No benchmark scores, no "typical" rates; the criteria in the sample goals are in brackets because your team sets them from the baseline and the student's IEP. Every probe is untimed on purpose and the goal pages say so, so the condition stays the same all year.
Who This Is For
Special education teachers and case managers writing K-2 level reading goals for any age student, intervention and Title teachers, and the paraprofessionals who give the weekly probe.
