Every accommodation in one menu, with a box to tick and a line for how it is provided, as a fillable PDF: a two-part accommodations menu, a testing accommodations checklist, a one-student teacher handout, a class roster, a quarterly check-in, and a filled-in example. What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not decide which accommodations belong on a plan.
It is not a 504 plan and it does not duplicate the meeting-prep, documentation, plan-writing, family, or review pages that a full plan needs — those live in the 504 Plan Writing Kit. It states no eligibility rule, no legal requirement, no timeline, and no statement of who qualifies for anything; wherever a rule would apply, the page says "as your district's plan states" and leaves a line to write it on. The example student is invented and says so on the page.
Who This Is For
Classroom teachers, case managers, and 504 coordinators who need the accommodations menu on its own — to select from it, hand a teacher only what applies in their room, keep a class-wide roster, and check in each quarter — without a full plan-writing kit.
