What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate rates or draw the graph.

It is not a device manual, a therapy program, or a list of strategies. The device evaluation sheets are structured templates, not clinical assessments. Nothing here says which cues to use, what counts as progress, or which device to buy; the team decides those from the IEP goal and the student's needs. Devices are named generically (switch, keyboard, tablet, text-to-speech reader, adapted seating). Whether the device stays, changes, or gets discontinued is the team's decision on the quarterly review page.

It is distinct from AAC Data Collection Sheets (17650310), which is built for communication devices specifically: core board, speech-generating device, communication book, and the communicative functions (request, protest, comment, greet, answer). This file covers general assistive technology — switches, alternate input devices, adapted seating, text-to-speech and reading tools, organizational technology, and anything that is not primarily a communication device. If you are tracking a student who uses AAC, that product is your match. If you are tracking a student using switches or switches plus other AT, adapted seating, a reading app, or an organizational tool, this is your file.
