About this resource
Blending and segmenting are the two oral building blocks under every early reading goal: can the student put sounds together into a word, and can they pull a word apart into its sounds? This pack measures both, every week, against an IEP goal you write. No reading or spelling is involved — it is entirely oral and untimed, one-to-one.
WHAT IS INCLUDED - 34 pages: 12 Blending forms and 12 Segmenting forms (24 total), each a teacher record sheet of 10 real single-syllable words, a goal page with a sample goal and baselines for each skill, a phoneme key, a binder cover and seven divider tabs, an IEP data sheet with a graph and log for both skills, a caseload summary, a family update and team handoff page, and a filled-in example
Blending
- The teacher says each sound with about a one-second pause between sounds (e.g. /m/ ... /a/ ... /p/); the student says the whole word
- Scored 1 point per word: correct only if the whole word is said
Segmenting
- The teacher says the whole word once; the student says every sound in it, in order
- Scored two ways on the same form: sounds correct (out of 38 per form) and words correct (out of 10 per form)
Goal, Phoneme Key And Directions
- A goal page with a sample goal covering both skills (every bracket meant to be edited), baseline tables for blending and for segmenting, a blank for your state standard and a blank for the district's own goal text
- A phoneme key on the How This Pack Works page: the five short-vowel sounds with an example word, and a note that sh/ch/th and ck are each one sound
Iep Data Sheet And Caseload
- One graph and log tracking both skills (words correct out of 10, with a sounds-correct column for segmenting)
- A caseload summary grid (one row per student per skill), a family update and team handoff page, and a filled-in example (Noah, invented)
What This Is, And What It Is Not
It is a fillable PDF. Every form, the goal page, the data sheet, the caseload page and the family update page are type-in fields: open the file in a PDF reader, click a line or a cell, type, and Save As once per student. It is an oral, untimed probe given one-to-one; it is not a norm-referenced assessment and it carries no benchmark score or expected rate. The criteria in the sample goal are in brackets because your team sets them from the baseline and the student's IEP. Your district's own rules for goals and scoring go on the blank lines provided. It is not a Google Slides, Docs or PowerPoint file.
How To Use It For Progress Monitoring
Give Forms A, B and C on each skill on three different days for a baseline (the middle score is the baseline), write the goal from the frame, then give one blending form and one segmenting form a week in rotation. Score each the same day, plot the point, and look at the last three points on each skill.
Who This Is For
Kindergarten, first and second grade special education teachers and case managers writing phonemic awareness goals, reading interventionists, speech-language pathologists, and the paraprofessionals who give the weekly probe.
