About this resource
Book sign out sheet for a classroom library, ready to type in or print. Three sign-out logs (by day, by student across the week, and by book), a shelf check, cut-apart student library cards and book pocket cards, a return reminder for home, a monthly roll-up and a rules poster, in one 20-page PDF.
EVERYTHING INSIDE (20 pages)
- 3 sign-out logs. The daily log has 16 lines for the date out, the student, the book title, the due back and the date returned. The weekly grid has 14 student rows and Monday to Friday. The book record has 16 rows, one for each book, marked In or Out with who has it.
- A shelf check with 8 named bins and 2 blank bins, counted against the book record, and a card for what to do when a book is missing.
- 14 cut-apart cards. 6 student library cards (My name, the book I am reading, the due back) and 8 book pocket cards that the teacher prints, cuts and puts in each book.
- A return reminder for home: 4 options with a box to check, the book's title and due date, a note back from the teacher, and a family signature line.
- A monthly roll-up with 11 student rows and a total row. Books out must equal back on time, plus back late, plus still out, and the page says to check it before you report.
- A rules poster with 5 rules, a picture for each, and a blank line for your school's own rule for a lost or damaged book.
- A phrase bank of 15 starters in 4 groups: for a return reminder, for a child who is stuck, for book talk, and for a missing book.
- A quick card for a para or a sub: the sign-out steps in order, the check-in steps, and what not to do.
- 2 filled examples for invented students: a daily log with 8 lines for 7 students, and the roll-up for the same lines with its sentence and check. A filled return reminder for a late book is on the log example page.
- A student reading record with 2 entries and a box to draw the book.
- Start Here with a table of which sheet to use when, the terms of use, a binder cover, a spine and 4 divider labels, a what's-next page and a credits page.
Built For The Child
The library card page and the reading record are for the child: each has a big "My name" line, one short instruction with a picture, and a star to color when done. Nothing on a child's page is written for an adult. The cards are cut apart by the child along straight, dashed lines, with 6 pieces edge to edge on one page.
Type In, Or Print
Every name, date, title and cell on the logs, the record, the shelf check and the roll-up is a type-in field (503 in all). Type in any PDF reader or print the page and write by hand. Every line is black or gray, so the pages copy in black and white. This is a fillable PDF; it is not a Google Slides or PowerPoint file.
Who This Is For
K through 5th grade classrooms with a library corner, bin or shelf of books to borrow: general education and self-contained rooms, paraprofessionals who sign books out, and substitutes who cover the library time.
What This Is, And What It Is Not
This is paper for signing books out and checking them back in. It is not a school library catalog or a school record system, and it carries no scoring rule and no goal text. The example students and book titles are invented, and the page says so. Your school sets what goes home and how; the rule for a lost book is yours to write on the blank line.
