About this resource
A work sample, a supervisor's note, a student's own words on a goal — most of it gets made, then lost, because there is nowhere for a student to keep it that a post-secondary program, a vocational agency, or an employer could actually read. This is that place: a student-facing portfolio that collects the evidence as it happens, lets the student reflect on it in their own words, and rolls the whole year up into a system a student can hand over or present at a meeting — set up once, filled all year, and built to drop straight into a binder.

- A portfolio cover sheet: career interests, three top strengths, one sentence about me, jobs I want to try, what helps me do my best work, and who I want to show this to
- A detailed work experience record (one page per job): tasks, supervisor feedback, what I learned, what I would do differently
- A work experience log (14 rows) — every job at a glance: employer, dates, hours, main task, whether a reference was collected
- A work sample record (11 rows) for describing a real artifact, photo, or object, the skill it shows, and evidence of growth compared to an earlier piece

What This Is, And What It Is Not
It is a fillable PDF: type into the tables and prompts, or print and use a pen. The binder cover, spine labels and divider tabs print like any other page and are meant to be cut apart. It works offline with no account or subscription. It is NOT an editable online slideshow or word-processor document, and it does not grade, score, or set a mastery or eligibility rule for any student.
The support-level mark (Independent / with a Cue / Direct Help) describes what was seen on ONE job or task — never a rule about the student. This is the student's own portfolio: an adult may write for a student who asks, but the words stay the student's. Nothing here states an age or grade as a rule, states a diagnosis, or gives legal or clinical guidance; your district's own rule for when this portfolio is reviewed gets a blank line to write it on.
