About this resource
Most picture cards ask a child for one word. These ask for a sentence. Every card prints a sentence frame ("I see a ___", "I want the ___", "The ___ is here", "I can ___") with a picture under it, so what the child says is a three- or four-word sentence built around the word the picture shows. The file gives you the frames, the pictures and a data sheet. Your own scoring rule decides what counts as correct, and every probe form has a line to write down exactly what was said.

Picture Cards
- 16 sentence cards in full color, each a big cut-apart piece laid edge to edge in a 4-across, 4-down grid, so cutting is a few straight lines. Each row is one frame: "I see a ___" (dog, cat, cow, bus), "I want the ___" (apple, banana, carrot, hat), "The ___ is here" (chair, bed, door, car), "I can ___" (run, swim, dance, clap)
- The same 16 cards again as an ink-saver set in pure black outlines, for the photocopier
- A note beside the cards naming the 6 pictures a child may name another way (dog, cat, hat, run, swim, dance), so you can decide once how those are marked
Probe And Track
- 3 quick probe forms (A, B, C) of 8 sentences each, two from every frame. Forms A and B split the 16 cards between them with no overlap; Form C is a fixed mix of 8 of the same cards (three forms of eight cannot all be different from a deck of sixteen), and every form has a "what was said" line
- Full-Deck Probe -- all 16 sentences, one mark each (+ / - / approximately), a verbatim column, and a tally for each frame
- Data Log -- 17 rows: date, form, correct, out of, percent, notes
- Percent Graph -- 14 probes across, 0-100 percent grid, a field for the team's own criterion
- Caseload Summary -- 16 rows: student, frame(s) practiced, baseline, current, trend, decision
Share And Write
- Practice at Home -- a family update with four of the same pictures and their sentences, a weekly practice table, and lines for the sentences a family heard outside of practice time
- Phrase Bank -- 18 observable-language starters across 5 groups (accuracy, what was said, carryover, the log and graph, the caseload page)
- Filled Example -- an invented student, eight probes, every percent re-counted from its own row, with a verbatim note that matches the form it came from
What This Is, And What It Is Not
It is a fillable PDF: type into the log, graph, caseload page, family page and probe-form headers, or print and use a pen. It works offline with no account or subscription. It is NOT an editable online slideshow or word-processor document, and it is not a norm-referenced or standardized test.
It gives frames and pictures. It carries no elicitation script, no cueing or prompting method and no modeling steps. It does not decide what counts as a correct sentence, a criterion, or a typical rate; each of those is your own call, written once and reused every session.
Who This Is For
Speech-language pathologists and special education teams with young or pre-reading students working on producing whole sentences, and the families who get the practice page sent home. If you already own Action Verb Naming Picture Cards or Category Naming and Classification Picture Cards, these pictures will look familiar; the new part is the printed sentence frame and the sentence data sheet.
