About this resource
Two teachers, one class, one planning period to split. This is the set of planning paper the inclusion pair writes on together, in one fillable PDF: a chooser across the six co-teaching arrangements, a partnership agreement, a weekly co-planning sheet, a lesson plan template for each arrangement (two-column, station rotation, parallel, alternative, team script), an accommodations-at-a-glance page for the class, a co-teaching data sheet, a roles and responsibilities checklist, a weekly debrief, a self-check rubric, and a filled-in example. Both teachers have a column on every plan. Type the class, the roster and the roles once, Save As per class, pen the notes during the lesson.

Five Lesson Plan Templates, Plus The Week
- Weekly Co-Planning Sheet - Monday to Friday by objective, arrangement, Teacher A role, Teacher B role, accommodations to build in, data to take; boxes for materials and for students to watch
- Single Lesson Plan, Two Columns - the template for one teach / one observe and one teach / one assist: six lesson parts (opening, model, guided practice, independent practice, check for understanding, close) by minutes, Teacher A and Teacher B; accommodations built in; materials; what to keep and what to change
- Station Rotation Plan - 4 stations by who leads, task, materials, group order and minutes; a 4-group name list with 6 rows each; the add-it-up line (opening + rotations x minutes + transitions + close = the period); the transition cue
- Parallel Teaching Plan - six parts (students, opening, approach, materials, check for understanding, accommodations built in) side by side for Group A and Group B; seating and noise; how the halves rejoin; the count from each half afterward
- Alternative Teaching Plan - eight parts (students, purpose of the small group, teacher, objective, what happens, materials, check, rejoining) for the large group and the small group; the pull criteria typed first; who led the small group on which date, so both teachers take a turn
- Team Teaching Script Plan - 13 turn-taking rows: minute, who (A, B or both), the move, and the transition cue that tells the other teacher to take over; the roles the pair takes; where the hand-off was late or early
Data And Review
- Co-Teaching Data Sheet - 12 rows: student, supported by, what the support was, what was measured and the number, where it goes (IEP goal, grade book, debrief); the whole-class check; students who need the small group next
- Weekly Debrief - two per page: what worked, what to change and who does it, students to watch, next week's arrangement and why, and one appreciation line from each teacher
- Co-Teaching Observation and Reflection - a self-check rubric: 8 areas by three levels the pair writes for its own room, a rating column, the evidence for the ratings, the one thing to work on, both signatures and the next check date
- A filled-in example for an invented pair, Ms. Cho and Mr. Diaz, grade 4 math: a three-station rotation for 24 students in three groups of 8, the roles for the day minute by minute (opening 6, three rotations at 15, three transitions at 1, close 6: 6 + 45 + 3 + 6 = 60, the period length on the plan), four accommodations built in with who provides each, and the data taken
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per class or per week. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add the minutes on the station plan; you add, it holds the answer.
It is not a curriculum and it is not a ranking of the six arrangements: they are described as ways to split a lesson between two adults, and the pair picks by what the lesson needs. Nothing in it says how a school has to grade, manage a room or schedule planning time; your school's own plan goes on the lines of the partnership agreement, and these templates are the working paper behind it. The accommodations page carries short forms and first names only; the plan and the case manager's file hold the rest.
Who This Is For
Inclusion co-teaching pairs: the general education teacher and the special education teacher who share a class, the paraprofessional who runs the third station, instructional coaches who set up co-teaching partnerships, and administrators who want one page (the rubric) to talk from when they visit a co-taught room.
