About this resource
CVCC and CCVC word lists for kindergarten and first grade, with the weekly probes to measure them. The lists are one-syllable words with one short vowel and a two-letter blend. CVCC words have the blend at the end (lamp, nest, hand, milk). CCVC words have it at the start (frog, step, clap, drum). Read the lists for practice, then use the reading forms as an IEP progress monitoring probe, with a goal page and a graph.
Every word has one short vowel and one blend. No word repeats across the lists and the forms, so the probe words never appear on a practice list.
WHAT IS INCLUDED - 23 pages: two word lists of 18 words each, one for blends at the end and one for blends at the start (36 words). Four untimed reading forms of eight words each, with four words of each kind on every form (32 words). A one-minute form of twelve words, six of each kind (12 words). 80 different words in all: 40 with the blend at the end and 40 with the blend at the start. A three-page answer key, a goal page, a how-to-read page for the student, teacher notes, a data log and graph, a caseload summary, a filled-in example, a family update page, a binder cover with divider tabs, a What's Next page, and a thank-you page with the terms. 356 type-in fields.
Two Word Lists (Pages 7-8)
- Page 7, blends at the end: camp, jump, band, west, and 14 more
- Page 8, blends at the start: clip, club, drip, grub, and 14 more
- A big My name line and one short instruction with a book picture
A How-To-Read Page For The Student (Page 6)
- Eight blend examples in two rows: four at the end, four at the start
- Three steps: find the short vowel, look for the blend, read the whole word
- Three worked words and two lines to write the student's own
Four Reading Forms, A Through D (Pages 9-12)
- Eight words per form, large type, each with a small number so you can mark errors
- A big My name line and one short instruction with a book picture
- No answers, goal text or scoring strip on a student sheet
A One-Minute Form (Page 13)
- Twelve words, six of each kind, for a timed check, with its own log on the graph page
The Goal Page And Binder (Pages 4-5)
- A sample goal with every bracket meant to be edited by the team
- The goal frame as type-in fields, a baseline table with three probes and the median, and three benchmark rows
- The giving-and-scoring directions, and lines for this student's own scoring rules
- A binder cover with student, case manager and school year fields, and six divider tabs to print and cut
A Three-Page Answer Key (Pages 14-16)
- Every probe word with its blend, so a paraprofessional can score without a list of their own
- A Right? box for each word and a Correct ___ / 8 box under each form
Teacher Notes (Page 17)
- Reading the errors: the end blend left off, the start blend left off, the blend letters out of order, a word guessed
- Which words are left out of the pack and why: two-syllable words, added endings, digraphs, and ng or nk endings
The Data Log And Graph (Page 18)
- A y-axis already scaled 0 to 8, twelve probe columns, and the instruction to draw the aim line from the baseline to the criterion first
- A nine-row log with date, form, correct, errors, the score used on the graph and notes, plus a separate five-row one-minute log
The Caseload Summary (Page 19)
- Twelve student rows, baseline and criterion, eight probe columns, a trend column and a decision column
A Worked Example (Page 20)
- Eli, first grade, invented. Twelve weekly probes out of 8: the baseline as a median, three points under the aim line forcing a teaching change, the criterion met, and the progress-report sentence the log produces
A Family Update And Team Handoff Page (Page 21)
- A family update with blanks for this week's score, a team handoff checklist, and a meeting summary sentence with blanks for two numbers off the log
What This Is, And What It Is Not
It is a fillable PDF. The goal page, data log, caseload page, family update page and the boxes on the answer key are type-in fields: open the file in a PDF reader, click a line or a box, type, and Save As once per student. The word lists and the student pages print clean; the student reads on paper. It is not a Google Slides, Docs or PowerPoint file.
Who This Is For
Kindergarten and first grade special education teachers and case managers writing a reading goal for blends at the start or the end of a short-vowel word, intervention teachers, and the paraprofessionals who give the weekly probe.
