About this resource
A daily note home, with a place for the family to write back. This is a fillable PDF for the special
education teacher or paraprofessional who sends a short sheet home every day: a simple three-step day
rating, a mood word, a win, a hard moment and what helped, and a reply box for the family. Nothing is
pre-filled about any child, and your district's own rule on what goes home gets a blank line.
WHAT IS INCLUDED — 18 pages: a detailed daily sheet, a quick two-per-page version, a morning check-in from
home, a week and a month at a glance, a staff notes page, a parent reply sheet, a cover letter to families,
note starters, a roll-up graph, a binder cover and tabs, and a filled example
Setup And Start
- Start Here — ten situations, one page each, plus the three steps
- How the Type-In Fields Work — one page, so nobody emails you
- Setup and Rating Key — first name only, how the sheet goes home, the optional lines you choose, and a
3 / 2 / 1 day rating key
- Cover Letter to Families — a welcome letter with blanks for your own wording
The Daily Note, In Real Variety
- Daily Communication Sheet, Detailed — six parts of the day with a rating and a note, a mood check,
optional meals / toilet / rest lines, a win, a hard moment and what helped, and a reply from home
- Quick Version — two days per page for a short note on a busy day
- Week-At-A-Glance — five days on one page with a rating, mood, note and home reply
- Month-At-A-Glance — 22 school days, each with a rating, one line and whether the family replied
A Second Audience And Setting
- Morning Check-In From Home — six questions the family can answer at drop-off
- Specials and Support Staff Daily Notes — 14 rows for the art teacher, the aide, the bus or a substitute
- Parent Reply and Conversation Sheet — a long reply box and a place to log a phone call
Reporting And Communication
- Daily Note Starters — 30 starters in six groups plus a blank box for your own, each ending in a blank
- Roll-Up — a five-week count of days sent, great, okay and hard days and replies, with a 20-day graph
- A Week of Daily Notes, Filled In — an invented second grader, one week shown with a three-week roll-up
whose every number is checked against its own table
- Binder Cover and Tabs — a cover, a spine insert and five divider tabs to print and cut
What This Is, And What It Is Not
It is a fillable PDF: open it in a PDF reader, click a line or a box, type, and Save As once per child. It
works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel
workbook, and it does not count or calculate for you.
It does not interpret a day, name a condition, or set a goal. A rating describes the day, and the team
decides what it means. It is not a speech-language data sheet; it is a daily note between school and home.
Who This Is For
Special education teachers, resource teachers, paraprofessionals, and early childhood classroom staff who
send a note home every day, and the families who write back.
