About this resource
A diagnostic reading assessment is only useful if you can see the results by skill and by child. This is the recording kit: five check sheets (letters, letter sounds, sight words, decoding words and fluency), each with a score box, plus a one-page profile for every student, a check-again page and a class summary. You type your own items once, give the check, and count what is right.
EVERYTHING INSIDE — 20 pages
- My Item Lists: 4 lists of 12 rows to type your own letters, letter sounds, sight words and decoding words, plus a fluency story block
- 5 check sheets, one for each skill: letters, letter sounds, sight words, decoding words and fluency. The four item sheets have 12 rows (item, R or M, what I heard) and a Right ___ out of ___ score box. The fluency sheet has a story block, 3 tries of words read, mistakes and words right, and a place for tricky words
- Each check sheet ends in a next-step box left blank for your own words
- My Reading Stars: a child page with five rows of 12 circles to color for each one right
- Reading Cards to Cut: 9 large cards in one block, each with a box to type one of your items
- Student Profile: Tried, Right and Missed boxes for five skills, a next-step line for each in your words, and places for what went well and items to look at again
- Check Again: four checks across, with a date row and one row for each skill
- Class Summary: 18 student rows by five skills, plus a date
- A note home with blanks in your own words
- A phrase bank of 32 starters in 8 groups
- A filled-in example: one invented child, Maya, and invented practice items, every count re-added from its own table
- A binder cover, a spine label and 8 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
Most reading check packs give you item lists. This one lets you use your own items, and then puts the results by skill, by child and by class on the page.
What This Is, And What It Is Not
It is a fillable PDF: type your items and counts into it, or print it and write by hand. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add up scores or copy numbers from one page to the next. You cannot drag the boxes around or restyle the pages.
There are no item lists, cutoffs, placement rules or grade-level norms in it. The items are yours. What a score means, which children get which help and what happens next are your school's rules, and the next-step lines are left blank for your words. The example uses invented practice items, not a list for any grade. No health information is filled in. The example child is invented and says so on the page. The cutting is the 9 reading cards and the 8 binder tabs, each in one block with straight cuts.
