About this resource
Gen-ed teachers, school counselors and digital-citizenship coordinators need to document a student's growing digital-safety skills and habits over time — not a one-time lesson, and not a scored test, but an ongoing record of what a student recognizes, knows and does online. This file tracks seven named digital-safety skills with a skill inventory, a whole-classroom snapshot, a weekly practice log, a scenario-based reflection with no score attached, a twelve-item cyberbullying-awareness checklist, a monthly roll-up and a quarterly graph.
This is NOT the School Counselor and Social Work Session Data Sheets pack (individual and group counseling session data, coping-strategy and consultation logs for a whole caseload) and NOT Student Self-Monitoring Checklists and Goal Sheets Editable (an in-the-moment behavior checklist for one IEP goal). It has one narrower job: seven named digital-safety skills, tracked from inventory to monthly report.
EVERYTHING INSIDE — 21 pages, 14 working pages, a filled example
- A digital safety skills inventory (7 rows, one per skill): password creation, recognizing phishing, privacy settings, identifying cyberbullying, appropriate online language, digital-footprint awareness, fact-checking online information — with a four-code rating scale (Emerging / Developing / Consistent / Independent)
- A classroom digital safety snapshot (13 student rows) using the same seven skills as one-letter codes, for a quick whole-class scan
- A digital citizenship lesson tracker (12 rows): what was taught, when, to whom, and which skill it addressed
- A weekly skill-practice log (10 rows): which skill, how it was actually applied, independent or prompted
- A scenario-based reflection: three built-in scenarios (a phishing email, a mean comment online, an oversharing moment) plus a blank for your own — explicitly NOT a test: no score, no norm and no right answer recorded, only what the student says
- A cyberbullying awareness checklist (12 items across three areas — recognize it, report it, respond to it) with a Y / N / Not Observed mark per item, plus a blank line for your own school's reporting policy
- A family online safety practice log (two 5-row logs per page, cut apart) — plain language, no codes, the second setting this file is built for
- A monthly digital safety profile (7-row roll-up, start-of-month vs. end-of-month rating per skill) with a 3-field summary card
- A quarterly progress graph (a 12-week plotting grid) plus a 3-row quarter-checkpoint table
- A digital safety family update, ready to send as printed or typed, including a 3-row skill-growth table
- A digital safety phrase bank: 21 observable-language starters across 7 groups (skill development, scenario responses, cyberbullying awareness, home carryover, lesson coverage, the family/home log, the monthly summary sentence)
- A binder cover and 4 divider tabs (organizer set)
- A filled example — an invented sixth-grade student, every count in the summary re-added from its own rows
- Start Here, a contents page with page thumbnails, and a "what's next" back page
What This Is, And What It Is Not
It is a fillable PDF: type into the inventory, the logs and the tables, or print and use a pen. The binder cover and divider tabs print like any other page and are meant to be cut apart. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or a word-processed file, and it does not score the scenario reflection or calculate a percentage for you.
A rating describes what has been directly observed — never a finding about the student and never a score on a test. The scenario page has no graded right answer anywhere in this file; it exists to start a conversation, not to record one. Nothing here sets a school or district's reporting policy for an actual cyberbullying incident; that stays a blank line for your own policy. Example students are invented and say so on the page.
Who This Is For
Gen-ed teachers, school counselors and digital-citizenship coordinators who need an ongoing, honest record of a modern skill set — plus the families this file brings into the same practice log at home.
