About this resource
WHAT IS INCLUDED - 18 pages, 1,137 type-in fields
Setup
- Which Sheet for Which Program - a chooser with nine rows that matches how the target is taught (one target to criterion, mixed targets, receptive and expressive, matching, imitation, the prompt level itself, maintenance, a whole day, a trend) to the sheet that records it, plus a table of how the paper moves each session, each day, each week and at mastery
- How the Type-In Fields Work - one page, so nobody emails you
- Code Key and Program Setup - the response marks, six prompt codes (I, V, G, M, P, FP) with a column to write what each looks like in this program, and a six-row setup block where the target, the direction as the team worded it, the criterion, the starting prompt level, the mastery rule and the maintenance schedule get typed from the team's own paperwork
- Program Roster - 12 rows, one per running program: student, program, target set, sheet page, criterion, started, mastered
Summary, Graph And Review
- Daily Session Summary - one student, one day, 8 programs by trials run, correct, percent, prompt level and notes, with a day total row and a day percent
- Cumulative Tracker and Graph - twelve sessions, 0 to 100 labeled percent correct per session, a date, percent and aim line row, and the aim line drawn at the criterion before the first point
- Weekly Program Review - 6 programs by sessions this week, this week percent, last week percent, trend and decision (continue, fade, mastered, revise), beside a four-row block where your team's own decision rules get their numbers written in
- A filled-in example for an invented kindergartner: a single target sheet with six sessions of ten trials, the two prompt fading rows those sessions produce, and the Friday review decision that came out of them
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate percents or draw the graph.
It is not a curriculum, a program book, or instruction in how to run anything. Your team writes the program; these sheets record it. The target, the direction as it is worded, the criterion, the prompt level to start at, the mastery rule and the maintenance schedule are typed onto the setup page from your team's own paperwork, and every sheet after that records what was already decided. Where a rule is needed, the page says "as your program specifies" and gives you a blank line for it.
Who This Is For
Special education teachers and case managers running trial-based programs, the paraprofessionals and behavior technicians who take the data all day, and the teams that need the same sheet filled the same way in every room and by every adult.
