One skill, start to finish. A sight word IEP goal for the Dolch Pre-Primer and Primer lists, six equivalent probe forms, and a graph scaled to the probe. Baseline on Monday, goal written by Tuesday, first data point on Friday.

This is the DOLCH list, not Fry: the store already sells Fry sight word reading probes for all three hundreds, and Dolch is a different published list some districts and goal banks name instead. Checked word-for-word against this store's Fry banks: 78 of these 92 Dolch words also appear somewhere in the Fry First, Second and Third Hundred combined (words 1-300), and 52 of the 92 appear in the Fry First Hundred alone (this store's original sight word probe) -- so a case manager who already owns that probe already has more than half this list, just not grouped, ordered, or labeled as Dolch. The other 14 words are on none of the three Fry hundreds at all -- 5 of the list's six color words (blue, red, yellow, black, brown) are on none of the three; white is the one exception, already in the Fry Third Hundred.

What This Is, And What It Is Not

It is a fillable PDF. The goal page, the goal box on every probe form, the data log and the caseload page are type-in fields: open the file in a PDF reader, click a line or a cell, type, and Save As once per student. The probe forms are meant to be printed; the student reads from paper and writes nothing. It works offline with no account or subscription. It does not calculate percents or draw the graph.

It is not a norm-referenced assessment and it carries no benchmark score or "typical" rate. The criteria in the sample goal are in brackets because your team sets them from the baseline and the student's IEP. Every probe is untimed on purpose; the goal page says so, so the condition stays the same all year.
