About this resource
A fillable, plain-language data-collection file built for a birth-3 Early Intervention caseload — home visits and routine-based practice, not a classroom. It tracks four domains (communication, motor, social-emotional, adaptive) through the outcomes the family's own IFSP already names, a routine-based observation log for meals, bathtime, play and sleep, and a caregiver-coaching note, because that is where Part C services actually happen.

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per child. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate a percent or draw the graph.
It is not a screening or eligibility tool, not a developmental-norms chart, and not a curriculum. No page states an age at which a skill should appear, and the checklist rates only what was actually observed. The four domains and every outcome are the ones the family's own IFSP already names — this file copies them, it never invents one. The caregiver-coaching page is a record of the visit, not a procedure; it does not say what a caregiver may or must do.
Who This Is For
Early Intervention (Part C, birth-3) providers — service coordinators, developmental therapists, SLPs, OTs, PTs and home visitors — who need progress-monitoring paper built for a home- and routine-based caseload, aligned to a family's IFSP. This is not the store's Preschool and Early Childhood Special Education Progress Monitoring Data Sheets (17720043), which is built for a PreK or group classroom caseload of 3-5 year-olds; and it is not the Paraprofessional Daily Data Sheets (17591667), built for the aide's own clipboard.
