About this resource
A dual-identified student — receiving special education AND English Language Learner services — has paperwork split across two teams and, usually, two folders. This file puts it in one: a services coordination note, language-domain progress in the home language and in English, an English proficiency growth log, a goal-by-domain summary, a cumulative tracker and graph, and a home note written plainly enough to translate. It is not the store's SLP file: for articulation and speech-sound therapy in two languages, see Bilingual Speech Therapy and Language Development Progress Tracking Data Sheets Editable instead — that file is built for an SLP's caseload. This one tracks general academic reading, writing, speaking and listening progress, the kind a case manager or an ESOL specialist reports on, not a speech sound. Every entry is dated; the file never scores proficiency, decides language dominance, or sets a proficiency scale — those stay blank lines for the district's own instrument and the team's decision.

Setup

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, press Tab to move on, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or a PowerPoint presentation.
It is not a proficiency test, a language screener, or a placement tool, and it carries no word lists or curriculum. It does not score proficiency, define language dominance, or set a proficiency scale — every one of those is a blank line for your district's own instrument and your team's decision. No age, eligibility, or clinical-assertion language anywhere; no health data is pre-filled, and the example student (Minh T.) is invented and named as such on page 17.
