The store already has a place to log every call, email, text and note home — that is a general communication log. This file answers a narrower question a general contact log was never built for: for ONE IEP goal, did the family get a home-practice activity, did they actually do it, how did the student do, and did the skill show up back at school. A case manager sets it up once per student, assigns the week's activity, logs each day with a one-letter code, and rolls the month up into the page an IEP meeting actually uses.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a completion percentage or a graph for you.

It is not the Parent Communication Log and Home School Notes Editable — the year-round record of every call, email, text and note home — and it does not replace it; use that file for the phone call and the concern conversation. This file has one narrower job: whether the home-practice activity assigned for a specific IEP goal happened, how often, and whether it carried back to school. It is also not IEP Goal Tracking Editable, which is the classroom side of the same goal — the trial data taken during instruction, its own tracker and graph. Use the two side by side for one goal: that file for what happened at school, this one for what happened at home.

It is not a therapy program, not a set of home activities, and not a rule about how much practice a goal needs. Which activity to assign, how often, and what counts as enough are the case manager's own judgment, written in from the IEP; this file never decides them on its own. No age, eligibility or IDEA-timeline claim anywhere; no health data is pre-filled, and the example student (Marcus) and his family are invented and named as such on page 22.
