About this resource
Most minimally-verbal and non-speaking students do not communicate one way. A gesture in the hallway, a sign at home, a device at school, a written note in therapy -- and a team that only counts the device misses most of the picture. This fillable PDF is one log for all of it: a multi-modality tracking grid, a setting observation log, communication function by modality, a requesting sheet, a modeling and prompting log, an integration and carryover log across settings, modality use across the day, a communication repertoire tracker, a partner check-in for the classroom, a quarterly summary of modality use, a tracker and graph, session notes, a family note, and a filled-in example. Type the roster, the codes and the sheet headers once, Save As per student, pen the marks in the moment.
Systems are named generically throughout: a core board, a speech-generating device, a communication book. No product, app or brand is named, so the sheets fit whatever your students use, in whatever combination.

Setup

Tracking, Reporting And Home
- Communication Partner Check-In - five days for the teacher or para: I modeled or prompted, the modality, I waited, the student responded, notes, and a reply line
- Quarterly Summary of Modality Use - one row per modality (5 rows): total messages, percent of total, settings observed in, and the trend
- Cumulative Tracker - 36 sessions in three blocks: date, measure, score

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate percents or draw the graph.
It is not an assessment tool, a therapy program or a modality-selection guide. It does not say which modality to teach, prompt or prioritize, and it does not screen, diagnose or rate anything against a norm. Nothing in it says what counts as mastery or carryover; you write those in from the IEP and your own judgment. The sheets make sure a message gets counted no matter which way the student said it.
Who This Is For
School SLPs, special education teachers and case managers with a functional communication goal that spans more than one setting or more than one modality, the paraprofessionals and general education teachers who see the student in a different room than the SLP does, and families asked to notice communication at home too.
