About this resource

The form goes out to five general education teachers three weeks before the annual review and comes back from two of them, or it comes back the morning of the meeting with one line per box, or it never comes back at all — and the present levels get written from a grade book instead of a classroom. This is the pack that fixes that: a one-page form most teachers will actually return, a longer form for the one teacher who sees the student every day, a way to ask out loud when paper is not going to happen, and one page that turns whatever comes back into notes ready for the table.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a slide deck, a word-processor file or a spreadsheet, and it does not email itself, remind anybody, or collect answers online. You type the header and the due date; the form is meant to be printed so the teacher can write on it by hand.

Who This Is For

Special education teachers and case managers writing annual and initial IEPs who need general education classroom evidence before they draft the present levels, co-teaching and inclusion teams who already share a classroom, and anyone who has run a meeting where a gen-ed teacher's inbox was still empty the morning of the review.
