About this resource

A weekly handwriting fluency probe, not another fine-motor rating sheet: the student copies a printed set of letters and words for a fixed, timed window, and a number goes on the graph. Four equivalent forms so the task changes every week and nothing is copied from memory, a scoring guide your team defines once, and a separate untimed motor checklist so a rising fluency score and a slipping formation quality never hide each other.

What This Is, And What It Is Not

It is a fillable PDF. The goal page, scoring guide, motor checklist, data log, caseload page and home note are type-in fields; open in a PDF reader, click a line or a box, type, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a score or draw the graph for you. The four probe forms print — a copying task is done by hand.

It is not an occupational therapy evaluation, a fine-motor or sensory screener, or a way to diagnose dysgraphia or any other condition. It does not decide what "correctly formed" means — your team writes that rule in once, on the scoring guide, before the first probe. No health data is pre-filled; the example student is invented and says so on the page.

Who This Is For

Special education teachers, resource room teachers and case managers who monitor a handwriting or written-output IEP goal and need a repeatable, timed measure instead of a one-time observation.
