What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box, or a cell, type, press Tab to move on, and Save As once per student or per cycle. It works offline with no account or subscription. It is NOT a Google Slides deck, a Google Doc, or a Microsoft PowerPoint file, and it does not calculate anything or interpret a result.
It is not a hearing test, not an audiogram, and not a diagnostic tool. No page states a decibel threshold, a pass/refer determination, an eligibility criterion, or a diagnosis — every screening-result field is blank for the screener's or audiologist's own finding, which stays in the student's health file. The accommodation pages describe what nine common supports look like in a classroom; they do not say which student should receive one. That decision, and the criteria behind it, is the IEP or 504 team's own, under your district's own policy.
Who This Is For
School nurses running a screening cycle, SLPs and case managers tracking auditory processing observations alongside speech and language data, and any special education team that needs the paperwork trail around a hearing screening in one file instead of scattered across sticky notes and email.
