About this resource
The record a teacher or case manager delivering homebound or hospital instruction needs when a file is reviewed or a team asks what has been happening: which sessions ran, how long, what was covered, and what went home — nothing about why the arrangement exists. Type the codes and the roster once, log sessions with a pen or a keyboard, and the weekly and monthly roll-up, the work-covered tracker, and the family or classroom-teacher update all copy straight from what is already on the page.
WHAT IS INCLUDED - 22 pages, 814 type-in fields
Setup
- Code Key and District Rules - 4 location codes (home, hospital or facility, virtual, other) and 5 reason codes for a session that did not run as scheduled, plus four blanks for your district's own rules
- Caseload Roster - 13 rows: student, subject or skill area, provider, assigned minutes per week, sessions per week, location, receiving teacher or school, start and review dates
- Binder Cover and Divider Tabs - a printable cover insert, four divider tabs (one per section of the file), and a before-the-first-session checklist
Log Every Session, Three Genuinely Different Ways
- Individual Student Session Log - 13 rows: date, duration, subject or skill covered, location code, what was covered in a few words, provider initials — the detailed, one-student, file copy
- Quick Session Log - 17 rows: time, student, duration, subject, location code, initials — the clipboard version for a provider covering several students or sites in one day
- Weekly Caseload Overview - 14 rows: student, subject, planned minutes, a day-by-day record Monday through Friday, actual total, and a missed/reschedule note — the whole caseload on one page
Roll It Up And Track The Work
- Weekly and Monthly Minutes Summary - 11 rows: student, subject, planned, delivered, missed, made up, balance, and a report note for the file or the meeting
- Work Covered and Assignment Tracking Sheet - 13 rows: date, subject, assignment or material given, given by, due/return date, returned Y/N, notes
- Missed Session and Reschedule Note - 13 rows: date scheduled, student, reason code, minutes missed, rescheduled date, minutes made up, who was notified
Communicate And Reference
- Family and Classroom Teacher Update - a ready-to-send page: subjects and skills covered this period (4-row table), work sent and returned (1-row table plus write-in), materials needed next session, a note in the provider's own words, and what this page is not
- Family and Receiving-Teacher Quick Reference - a second-audience page: at-a-glance provider and contact fields, a fill-in weekly schedule (4 rows), a who-to-contact table (2 rows), what to expect, what is not on this page, and space for notes
- Session Note Phrase Bank - 21 observable-language starters in 6 groups (what was taught, engagement and completion, work sent and returned, a missed or rescheduled session, the family/teacher update, and a group written for the receiving classroom teacher)
- A filled-in example for an invented student, described only as "a student receiving homebound instruction": a week of session-log rows, the missed-and-made-up arithmetic, the work-covered rows, and the minutes-summary row it all rolls up to
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student or per month. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not add the minutes or fill in a schedule for you.
It is not a record of why homebound or hospital instruction was arranged, not a health or status record, and not a timeline for when instruction will end — none of that is anywhere in this file. It records instructional time and content only: what was taught, for how long, where, and what was communicated. Every rule about make-up sessions, reporting cadence, or what a receiving teacher is told beyond what was taught is your district's own policy, written on a blank line.
Who This Is For
Teachers and case managers delivering or coordinating homebound or hospital instruction, the classroom teacher or school receiving a student back into a building setting, and families who want one page that says what is happening and who to ask.
