About this resource
One agenda per kind of IEP meeting, as a fillable PDF: annual review, initial IEP, re-evaluation review, amendment, and a transition-focused agenda for a secondary student. Each has a time, a leader and a notes column per item, and the order column is left blank so you number the items the way your team takes them. Around them: a plain-language parent copy of the agenda, a before-the-meeting checklist, a during-the-meeting checklist for the facilitator, an after-the-meeting checklist, a sign-in and roles page, and a filled example whose times add up to the meeting length.

- Annual Review Agenda - 12 items down, with a blank order column, a time column, a leader column and a notes column; the family speaks before any staff report and the decisions are read back before anyone signs

- Re-Evaluation Review Agenda - 12 items: existing data, new results by area from the person who gave each one, and the goals that stay, change or end
- Amendment / Addendum Meeting Agenda - 8 items for one change, a box for the change in one sentence, a line for your district's rule on whether the change needs a meeting, and a 4-row table of who receives the amended pages
- Transition-Focused Agenda - 12 items for a secondary student: the student's own goals first, strengths and preferences, where they are now, classes and what they lead to, transition services, outside agencies

Three Checklists And The Sign-In
- Before the Meeting Checklist - 20 tasks with a tick box, an owner and a done-date column; no dates are printed, and your district's notice rule gets a blank
- During the Meeting Checklist - 16 items for the facilitator: introductions, purpose, family input first, each item kept to its time, decisions read back, the sign-in page, next steps, with a notes column and a card for a meeting that is running long
- After the Meeting Checklist - 14 tasks with an owner, a due date and a done date: copies sent, teachers and providers informed, services scheduled, data sheets started, file audited
- Meeting Sign-In and Roles - 12 rows: name, role, attended full or part with the times, initials; plus a decisions-made box on the same page, and a note that the district's signature page is the official record

- A filled-in annual review for an invented sixth grader: twelve timed items that add up to the 60 minutes written in the header, and the before-meeting checklist exactly half done, with dates on the ten finished tasks
What This Is, And What It Is Not
