Before The Meeting
- Meeting Prep Countdown - weeks out, one week out, the day before. Fill in the target date for each row from the timeline your district requires.
- Parent and Family Input Form - strengths, what is working, what is worrying them, what works at home, what they want a year from now, the questions they want answered. The most useful page in the file.
- Student Input Form - in their own words or with an adult writing for them, with a line asking whether they want to attend.
- Teacher and Provider Input Form - one per adult who works with the student, including which accommodations are actually used and which are only on paper.
- Draft IEP Review Checklist - present levels, goals, services and accommodations, audited before anyone else sees the draft.
During The Meeting
- Meeting Agenda - eleven components in the order they are usually taken, with a planned-time column.
- Meeting Notes - family concerns first, then decisions, then action items with owner and date, then disagreements and the parking lot.
- Attendance and Participation Record - a working record for your file, including interpreter and translation needs.
- Explaining Rights and Consent in Plain Language - seven things to say out loud so the procedural safeguards notice is not the only place the information lives.
Plus the agenda and notes pages FILLED IN for an invented student, so you can see the level of detail that makes the notes useful three months later.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line or box, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Slides deck, a Google Doc, or a PowerPoint file. Prefer paper? Print it blank and handwrite it - the input forms that go home are meant to be handwritten by the family.
It is working paper, not the legal record. Nothing here replaces your district's signature page, prior written notice, consent forms, or procedural safeguards notice - those are the official documents, and the pages that could be confused with them say so on the page. Timelines vary by state, so no page names a legal number of days; every date column says to work backward from the date your district requires.
