About this resource

Carrying It To The Meeting

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a slide deck, a word-processor file or a spreadsheet, and it does not translate itself, email itself, or collect answers online. You type the header and the return date; the form is meant to be printed so the family can write on it by hand.
It is not your district's form, a notice of meeting, a consent form, or a statement of anyone's rights. Nothing here names a timeline, an eligibility rule, an age, or a label. Where your district decides something - when planning for after high school begins, who arranges a translated copy or an interpreter - the page gives you a blank line to write your district's own rule on.
