About this resource
"Struggles with reading" is not a present level. "Reads 42 words correct per minute on a second-grade passage in May; the benchmark is 90" is.
This kit turns data into the sentences the present levels section needs, as a fillable PDF. Fill the data-to-statement worksheet, pick the frames for the domains that apply, write the impact paragraph with the four-move frame, run every need through the bridge so it becomes a goal or a written reason, assemble, and check it against twelve questions before it goes in the draft. A complete worked example, annotated in the margin, shows what finished looks like.
WHAT IS INCLUDED - 20 pages, fillable
The Worksheets
- Data-to-Statement Worksheet - one row per piece of evidence: source and date, the number, what it means for access, the sentence for the draft.
- Strengths and Interests Organizer - from the family, the student, and the data, to three strength sentences the goals can be built on.
SENTENCE FRAMES BY DOMAIN - six pages
- Reading - decoding, fluency, comprehension
- Writing - mechanics, composition, process
- Math - number sense, computation, problem solving
- Communication and Language - receptive, expressive, pragmatic
- Behavior and Social-Emotional - what it looks like, what it costs, what works
- Functional, Adaptive, Motor and Attention - self-care, organization, attention, motor
Every frame has a slot for the number, the source, the date and the comparison, plus a strength frame, an in-class frame and a need frame. Lines under each frame are type-in fields.
The Hard Paragraphs
- How the Disability Affects Involvement and Progress - the four-move frame (the class requires / right now / so without support / with), a worked version, and lines for yours.
- Needs-to-Goals Bridge - every need becomes a goal, an accommodation, a service, or a written reason. Nothing gets named and dropped.
- Parent and Student Input Paragraph - their words, organized, so the family's priorities show up later in the IEP.
Finishing
- Assembling the Section - the six-part order that reads well.
- Self-Check Before It Goes in the Draft - twelve yes/no questions with a fix-needed column.
- A Complete Present Levels, Annotated - an invented second grader, every sentence explained in the margin.
- Words to Avoid, and What to Write Instead - fourteen adjectives that feel like present levels and are not, each with the sentence a stranger could use.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Slides deck, a Google Doc, or a PowerPoint file. Prefer paper? Print it blank and handwrite it.
It is not a legal template and it does not say what your state requires the section to contain. Your district's IEP form decides the headings; this kit decides what goes under them. The worked example is an invented student with invented scores.
Who This Is For
Case managers, special education teachers, intervention specialists, and anybody who has stared at a blank present-levels box at 9pm and would like the sentence to write itself from the data.
