Seven Half-Page Cards, Two Identical Cards Per Page
- Student Snapshot Card - first name, grade and teacher, case manager, updated; strengths; goals in three words each; a three-row accommodations table (short form, what the adult does); what helps; what to avoid; who to call; the one thing an adult must know today, as written in the plan
- Substitute Card - what this student needs from you today in order, a four-row routine (time, where they go, who takes them), the signal that it is going wrong, what to do first, the person to call, and where to return the card before you leave
- Specials / Related Arts Card - for art, music, PE and library: a three-row table of the accommodations that apply in that room, how the student communicates and how to respond, safety notes as written in the plan, how the class starts and ends for this student, what helps in a loud or moving room, who to call
- Paraprofessional Support Card - a four-row schedule of support (time, setting, what you do, fade plan), the prompt level that counts as independent per goal, the data to take and where the sheet goes, what helps, what to avoid, who to call, when the data sheet is due
- Bus, Lunch, Recess Card - for the settings with no classroom: what this student needs from you here, what helps, what to avoid, how they tell you something and how you answer, two things to do in a hard moment, who to call, where the student goes next
- Testing Accommodations Card - a four-row table of assessment accommodations in short form with the setting, the timing and who provides each; what is not allowed on this test per the test rules; breaks; materials to have ready; proctor; the date it was confirmed with the test coordinator
- Communication Card - how the student communicates, the device or system as written in the plan, how to respond (wait time, what to say, what to model), a three-row table of what a signal usually means and how you respond, what a refusal looks like, where the device lives and who fixes it, the student's words or signs for help, done and break
Caseload, Daily And Log
- Caseload Snapshot Grid - 12 students on one page: first name, grade, the key accommodation in short form, service minutes per week, which cards exist, review date
- Who Needs What Today - a daily half-sheet for the co-teacher, two per page: 8 students down, the three supports that matter today across, and who provides each
- Card Index and Distribution Log - 14 rows: student, card, which adult it went to, date given, replaced on, returned, destroyed at year end; with the eight card short names and their page numbers

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. Both cards on a page are identical, so either half can be handed out. It is NOT a Google Slides deck, a Google Doc or a PowerPoint file, and it does not become one.
It is not the IEP and not a summary of the IEP. A card says what helps, what to avoid and who to call; no card in this file has a place for a diagnosis, a category or a medical history, and every card carries a first name only. Whether a card may leave the building is your district's rule, and there is a line to write it on.
