About this resource

A request for an interpreter comes in by phone, by email, or at the front desk, and by the time it is confirmed nobody wrote down who asked, who was asked, or whether it ever got answered. This file is the log that catches that: one row per request, one sheet per confirmed meeting, one row per document sent out for translation.

This is a scheduling and tracking tool, built as a fillable PDF, for the case manager or coordinator who lines up interpreter support for IEP meetings, family conferences and calls, and who tracks documents sent out for translation and back. Type it once at the start of the year, and it is fast with a pen or a keyboard after that.

Plus a binder cover and three divider tabs that print straight into a shared binder or a front-office folder, and a filled worked example: an invented coordinator's September — six requests, five confirmed, four languages served, four documents sent and three returned — with every number in the summary sentence checked against the rows above it.

It is a fillable PDF: click a line, a box or a cell, type, and Save As. It works offline, with no account or subscription, and prints the same way it types.

It is a scheduling and tracking log only. It is **not** a consent form, and it does not describe, promise or decide what language support a district provides or how a request gets prioritized — every page that touches district policy leaves a blank line for your district's own wording instead of a finished sentence. No law or regulation is named anywhere in the file. It is not a credential record or a supervision log for an interpreter, and every name in the worked example is invented and says so on the page.

Special education case managers and coordinators who schedule interpreter support and track translated documents, and the front-office staff who often field the request first.
