About this resource

Which iReady lessons a student has passed, by subject (Reading and Math) and by week or month. This is a plain lesson log, not a probe and not the iReady program itself: there is no scoring, no lesson content, and no connection to an iReady dashboard anywhere in this file.

Six Tracking Sheet Formats

• Individual Weekly Lesson Log by Subject - 18 rows, lesson by lesson: week, subject (Reading or Math), lesson name/skill, pass or not yet, notes. The detailed, one-student version.
• Individual Monthly Lesson Log by Subject - 15 rows, rolled up by month instead of by lesson: month, subject, lessons passed, lessons not yet, notes. The big-picture version for a long term.
• Quick iReady Check-In Log - 22 rows, one row per check instead of per lesson: date, subject, pass or not yet, running total. The fast version for when there is no time for lesson-by-lesson detail.
• Small-Group iReady Lesson Log - 14 rows, one shared lesson list, 4 students tracked side by side as each lesson is finished.
• Caseload iReady Lessons Overview - 18 rows, one per student: subject, lessons passed, lessons not yet, last updated. The whole caseload at a glance.
• Home iReady Practice Log - 10 rows for a family to log at-home practice minutes between school check-ins (no pass marks on this page by design).

Reporting And Communication

• iReady Progress Snapshot - 12 rows, review period by review period: lessons passed this period, running total, Reading/Math split, trend. No graph: counts only, for a meeting.
• Family and Team Update - a short note naming the lesson or subject a student is working on right now, lessons passed since the last update, and what is planned next.
• Observation and Notes Phrase Bank - 18 observable-language sentence starters across five categories (lesson engagement, lesson results, conditions and support, patterns across several lessons, progress toward the goal), every one ending in a blank for what was actually seen.
• A Tracking Sheet, Filled In - an invented fifth grader, five lessons (including one retry), the weekly log, the monthly rollup, and the progress snapshot all built from the same rows, with the counts shown.
• Binder Cover and Divider Tabs - a cover page and four divider tabs (Setup, Tracking, Summary, Home) to print, cut, and drop into a three-ring binder.

What This Is, And What It Is Not

It is a fillable PDF: open it in a PDF reader, click a line or a cell, type, and Save As once per student or group. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc, or an Excel workbook, it does not connect to an iReady dashboard, and it does not mark lessons passed for you.

It is NOT the iReady platform itself, and it does not reproduce iReady's lesson content, scores, or skill sequence. It is a standalone tracking document for use alongside iReady. Every lesson name, subject, and pass field is blank for you to fill in from your own iReady dashboard and your school's own rules.
