About this resource

Everything a DHH itinerant teacher, a vision itinerant teacher, an itinerant SLP, or any related-services provider who drives between buildings needs to run the route, in one fillable PDF: a multi-site caseload roster, a weekly schedule grid across every site, a site-to-site mileage log, a monthly mileage-and-visits roll-up, a note that introduces the schedule to a building, a front-office reference card, a new-site orientation checklist, a missed-visit coverage note, a phrase bank, a binder cover and divider tabs, and a filled example across three invented sites. Type the setup pages once, fill the grid and the log as the week happens.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, and Save As once per year or per site. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not subtract odometer readings, add a mileage column, or calculate a reimbursement amount — the totals rows are added by hand, and the district's own mileage rate and submission process decide any reimbursement.

It is not the Paraprofessional Schedule and Zone Plan (a single building's room-level para assignments) and not a record of what any provider is required to do. It is not a health record and it does not decide a student's service minutes or eligibility — the team's own decisions and the district's own policy get a blank line to write them on. The example provider, sites and students are invented and say so on the page.

Who This Is For

DHH itinerant teachers, vision itinerant teachers, itinerant SLPs, and any related-services provider who covers more than one building on a fixed route and needs the schedule, the caseload and the driving logged on paper.
