About this resource

Every data sheet a PreK or Early Intervention team needs to track kindergarten readiness during transition, in one fillable PDF: readiness assessment across five domains (social-emotional, communication, fine and gross motor, self-care and adaptive, academic pre-skills), a caseload roster, five readiness checklists, a tracker and graph showing progress toward kindergarten entry, a transition meeting summary page for the team, a parent readiness survey, a phrase bank for transition notes, a filled-in example, a home letter template, and organizer tabs for a transition binder.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per child. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate or draw the graph.

It is not a kindergarten-entry requirement, a developmental screening tool, or a prerequisite for school entry. Nothing in it states an age at which a skill should appear or an age cutoff for kindergarten. Readiness skills are ones the IEP team has identified for this student's transition to kindergarten; the sheets make sure the team writes down the readiness data the same way, every time. This tool is built specifically for transition planning, not general developmental tracking.

Who This Is For

PreK and Early Intervention teachers, special education coordinators, and kindergarten teachers tracking and planning the transition to kindergarten entry. The tool is built for the transition period (typically late PreK spring through early kindergarten fall), not general preschool monitoring.
