About this resource

A fillable language screening record for SLPs: 30 things to listen and look for across understanding, talking and social language, two big-print student pages, a quick look, a conversation sample, a classroom observation log, a summary with blanks for your district's own rating scale and criterion, a team decision record, teacher and family input forms, a family summary letter, a tracking log, a phrase bank and a filled-in example. Type the headers once, Save As per student, and pen the marks.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not score anything.

It is working paper, not a screening procedure. There are no age expectations, no cutoff scores and no labels in this file. The rating scale, what counts as a concern, who is screened, and what happens next are your district's rules, and each has a blank line to write it on. The recommendation and decision pages offer checkboxes and a line for your district's own wording. The example student is invented and says so on the page; no real student data is included.

Who This Is For

School-based SLPs who screen students' language, SLP assistants and clinical fellows working under them, and special education teams that want one consistent record in the file. The teacher and family pages let classroom staff and families add what they see and hear.
