About this resource
Every data sheet a life-skills classroom or a job coach needs, in one fillable PDF: two task-analysis layouts with a prompt code in every cell, a daily living routine checklist, a self-care inventory, domestic and classroom jobs, community outings, work tasks, a money and time probe log, a generalization check, a tracker and graph, a quarterly summary, a home practice note, and a filled-in example. Type the routine steps and the headers once, Save As per student, pen the prompt codes.
WHAT IS INCLUDED - 21 pages, 1,375 type-in fields
Setup
- Which Sheet for Which Life Skills Goal - a chooser that matches the goal's wording (steps, a daily routine, a job, an outing, a work task, a probe, generalization) to the sheet that measures it, and a table of how the paper moves each session, each week and each quarter
- How the Type-In Fields Work - one page, so nobody emails you
- Code Key - a six-level prompt hierarchy (I, V, G, M, PP, FP) with a column to write what each level looks like for this student, four independence levels and four quality ratings the team defines, four setting codes (school, home, community, worksite), and five progress status codes for the quarterly summary
- Life Skills Caseload Roster - 13 rows: routines being taught, how each is measured, which sheet, the setting, IEP date, review due
Nine Data Sheets
- Task Analysis Data Sheet - up to 12 steps you type, 8 sessions across, a prompt code in every cell, with date, setting, steps-independent and percent rows
- Task Analysis Data Sheet for Short Routines - 6 steps by 12 sessions, so a week and a half of a short routine sits on one line and matches the graph
- Daily Living Routine Checklist - two routines a page (arrival, hygiene, a meal, clean-up), 7 steps each, five days across, a prompt code per step per day, steps-independent and initials rows
- Self-Care Skills Inventory - 16 skills you name, rated not yet / with help / independent, with date observed, setting and notes
- Domestic and Classroom Jobs Data - 12 rows: job, steps completed, most help needed, quality 1 to 4, time taken
- Community Outing Data Sheet - 10 rows: destination, skill targeted, opportunities, independent, prompted, prompt level, safety notes in observable words, plus a box for your district's outing rule
- Vocational / Work Task Data Sheet - 12 rows: task, units completed, time, accuracy, prompts, work-behavior rating 1 to 4, and a box for what the worksite supervisor said
- Money and Time Skills Probe Log - 13 rows: skill probed, items given, correct, percent, setting, prompt; the items come from your own materials and the page says so
- Generalization Check - 12 rows: skill, three settings across with an I or a prompt code in each, dates, independent in all three, who observed
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not calculate percents or draw the graph.
It is not a curriculum, a list of routines, or a prompting procedure. The steps of every routine are typed by you; the prompt hierarchy, the independence levels, the quality ratings and the setting codes are written in the way your team defines them. Nothing in the file says which prompt to use, when to fade it, or what a student should be doing at any age. The money and time probe log records probes given with your own materials; it contains no coins, prices or clocks. Whether an outing needs a permission form or a particular ratio is your district's rule, and there is a box to write it in.
Who This Is For
Self-contained and life-skills special education teachers with daily living, domestic, community or vocational goals on the caseload, the paraprofessionals who take the data at the sink and on the bus, job coaches and transition staff who need the same sheet used the same way at every worksite, and case managers who write the quarterly report from it.
