The store carries reading comprehension and a general speech-therapy data-sheet pack, but nothing for LISTENING comprehension on its own — a different skill from reading comprehension, because the passage is read aloud by the adult and the student never sees the text. This file gives an SLP or interventionist three original, invented passages built for that purpose, a rubric that scores each answer correct, partially correct or incorrect (never a percentile or age-based score), an observation checklist for listening behaviors that stays out of clinical labels, and a check for whether comprehension holds up across one sitting instead of only across separate weeks.

What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, save. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a percent or draw the graph for you. The three passages print for the student's ear only — nothing on the Passage Bank page needs to be a field.
It is not a norm-referenced or standardized test, and it is not a screening tool for any clinical category. The observation checklist records what was seen, never why, and does not sort a student into a label — a team concern about a pattern belongs to your district's own procedure, on a blank line this file provides. It is not Speech Therapy Data Collection Sheets Editable, the full SLP caseload pack (articulation, language trials, group sessions and more) — use that file for the rest of a caseload's data and this one when listening comprehension needs to be measured on its own. It is also not IEP Goal Tracking Editable, the classroom teacher's general goal tracker; the two pair well for a student whose IEP names both a listening goal and a classroom goal.
No age, eligibility or IDEA-timeline claim anywhere; no health data is pre-filled, and the example student (Imani) is invented and named as such on page 23.
