About this resource

The store already carries a Reading Intervention Data Sheets file (17649760) and a Math Intervention Progress Monitoring file (17728240, the small-group math side of this shelf) — this file adds what neither carries for LITERACY: a group-composition and rationale page (who is grouped with whom and why), a group-wide reading-level advancement tracker, and a shared-goal progress graph comparing one student's response with the other three at the same table. Form the group, take one data point per student per session on the page that fits it, and let the loop turn the numbers into a decision.

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As per student or group. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a percent or draw the graph for you.

It is not a reading program, not a leveling system, and not a placement tool. It does not say which students belong in a small group, does not name or teach a strategy, and does not set an accuracy, fluency or level target anywhere — every place a threshold, a criterion, or a leveling system name would go says "as your program specifies" or "as your team specifies" and gives a blank. It does not decide when a student or a group moves to less-intensive instruction; that decision, and the rule behind it, is your team's own, written on the line the exit sheet provides. No age, eligibility, or IDEA-timeline claim anywhere.
