About this resource

The store already carries a Reading Intervention Data Sheets file (17649760); this is the math side of the same shelf — a data-collection file for a small-group MTSS/RTI math intervention, not a curriculum. Set the skill focus once, take one data point per student per session on the sheet that fits it, and let the weekly loop turn the numbers into a decision.

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As per student or group. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it does not calculate a percent, a rate, or draw the graph for you.

It is not a math program, not a set of strategies, and not a screener. It does not say which students need math intervention, does not name or teach a strategy, and does not set an accuracy or fluency target anywhere — every place a threshold, a criterion, or a program name would go says "as your program specifies" or "as your team specifies" and gives a blank. The fidelity checklist checks a session against the plan your team wrote for it, not against any outside standard. It does not decide when a student moves to less-intensive instruction; that decision, and the rule behind it, is your team's own, written on the line the exit sheet provides. No age, eligibility, or IDEA-timeline claim anywhere.
