About this resource
One math screener for the whole intervention caseload. Grades 3 through 8, twelve original mixed-skill problems a grade, an answer key for every grade, a score sheet and a class record, so you can see which skill areas to teach next without building anything yourself.
Each grade has its own color and two student pages of six problems. Every problem has a dashed box to show work and an answer line, so the page goes straight to the student. Every grade covers the same six skill areas, two problems each: computation, fractions and decimals, place value and number sense, equations and expressions, measurement and geometry, and word problems.
WHAT IS INCLUDED - 24 pages: 6 grade pages (grades 3, 4, 5, 6, 7 and 8) of 12 problems each, 72 problems in all, on 12 student pages; 2 answer-key pages covering all six grades; a Start Here page, a skills map, a score sheet, a class record, a filled example, a family note, a binder cover with spine and divider tabs and a what's-next page; 314 type-in fields
Start Here And The Skills Map (Pages 3-4)
- The five-step routine, and a table showing the two problems each grade asks in each skill area
Six Color-Coded Grade Pages (Pages 5-16)
- Grades 3 to 8, two student pages each, six problems a page, one problem of each skill area on each page
- A big My name line, one short instruction with a pencil picture, a dashed work box and an answer line for every problem
- No answers or scoring on a student page
Answer Keys For Every Grade (Pages 17-18)
- All 72 answers, the skill each problem checks, and a right/wrong mark box beside each, three grades to a page
Score Sheet (Page 19)
- One student: grade page used, supports given, right answers in each skill area (out of 2) and in all twelve, space for what you notice, and a blank for your district's rule
Class Record (Page 20)
- Eighteen student rows with the six skill areas, the total out of 12 and a next-step column, for grouping
A Filled Example (Page 21)
- An invented grade 5 student, tallied by area from the answer key, with the arithmetic shown
Family Note And Team Handoff (Page 22)
- A note home with blanks for your own words, and a handoff checklist
Binder Cover, Spine And Divider Tabs (Page 23)
- A binder cover with fields, a spine label, and ten divider tabs (Start Here, one per grade 3 to 8, Answer Keys, Score Sheets, Class Record)
What'S Next (Page 24)
- Where to go after the screener: a short single-skill check on the low area, and how to group students
What This Is, And What It Is Not
It is a fillable PDF. The score sheet, class record, family note and answer-key mark boxes are type-in fields: open the file in a PDF reader, click a line or a cell, type, and Save As once per student. The grade pages print: the student writes on paper.
It is a classroom check for planning teaching. It is not a standardized test, it has no norms or cut scores, and it does not decide eligibility for any service. The problems are original and are not copied from any published test. What a score means is your district's rule, and the score sheet has a blank line for it. It is not a Google Slides, Docs or PowerPoint file.
Who This Is For
Special education and intervention teachers, case managers and general education teachers in grades 3 to 8 who want a quick look at where students are across several math skills, and a way to see a whole group on one page.
