About this resource
Everything inside: 24 pages · six clean student maze passages (18 three-choice blanks each, 108 blanks in all) · a shared scoring-keys page for all six forms · a data log and graph scored as correct word choices per minute · a caseload summary · a quick reference card for a second adult · a family update · an 18-starter phrase bank · a binder cover and 4 cut-out divider tabs · a worked, invented example.
A maze task is the progress-monitoring way to check comprehension WHILE a student is reading, not after — every seventh word of a passage is replaced with a box holding three words (the one that belongs, plus two plausible but wrong choices drawn from the same reading-level word list), and the student reads silently and circles the word that keeps the sentence making sense. That is a different skill from oral reading fluency (decoding speed, read aloud) and from an after-reading comprehension quiz (recall after the fact) — this store has both of those, but nothing that checks comprehension in the moment, word by word, while the student is still reading. Six equivalent passages, scored under a time limit your team sets, as a rate and accuracy together — correct word choices per minute (CWCPM) — never accuracy alone.
WHAT IS INCLUDED — 24 pages, 348 type-in fields
Set Up
- Binder Cover and Divider Tabs — a spine/cover card, 4 cut-out tabs, and an 8-row "where to find things" index
- How This Pack Works — six numbered steps from baseline to weekly routine to reteach decision
- Maze Reading Comprehension — Goal and Directions — a sample goal with a six-part fill-in frame, a 3-probe baseline table, a 3-row benchmark table, and scoring directions including the CWCPM (correct word choices per minute) formula
Six Timed Student Passages, Plus A Shared Key
- Forms A-F — one passage each (grade 3-4 reading level), 18 three-choice blanks a passage in reading order, each page clean for the student: a big "My name" line, one short instruction with a drawn book icon, and nothing else an adult would need to read
- Scoring Keys — the correct word for every blank on all six forms, laid out in the same reading order as the student page, plus a quick score strip and the scoring formula — kept off the student page entirely
Track, Report And Share
- Maze Reading Comprehension — Data Log and Graph — a 12-row log (date, form, time, reached, correct, errors, CWCPM, notes) and a graph scaled 0-20 CWCPM
- Caseload Summary — 12 students by 8 probes, one decision column
- Quick Reference for a Second Adult — six numbered steps, a "what not to do" list, and a mini scoring strip to hand back, for whoever gives the probe when it is not the case manager
- Family Update — a ready-to-send communication page: what was practiced, one win, a home-practice idea, this week's rate in plain words, and a 7-day practiced table
- Progress Note Phrase Bank — 18 observable-language starters across 5 groups (speed/accuracy, pattern across probes, next steps, quarterly report sentence, caseload-wide)
- A Filled-In Data Log — a worked, invented example (Devon, fourth grade) with 12 weekly timed probes, every CWCPM and accuracy number checked against its own row
Plus a Start Here quick-start, a 3-section contents page with real page thumbnails, 3 section dividers, and a "what's next" back page.
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per student. It works offline with no account or subscription. It is NOT an editable online slideshow or word-processor document, and it is not a norm-referenced test — it carries no fixed comprehension benchmark score anywhere in the file. The six student passage pages and the scoring-keys page print; the goal page, log, caseload page, second-adult reference and organizer are type-in.
It is not a curriculum, and it is not a rule about how many word choices per minute counts as "comprehending." What counts as enough for a given student is the team's own judgment, written in from the baseline and the IEP; this file never sets that number on its own. No age, eligibility or IDEA-timeline claim anywhere; no health data is pre-filled, and the example student (Devon) is invented and named as such on the worked-example page.
Who This Is For
Reading interventionists, special education teachers and case managers who need a timed, repeatable way to track reading comprehension while a student reads — not just whether they can answer questions afterward — plus a second, condensed page for whoever else gives the probe.
If You Want More
Oral Reading Fluency Grade 3-4 Probes is the timed read-aloud decoding companion, built on the same six passages at the same reading level. Main Idea and Supporting Details, Cause and Effect, and Compare and Contrast Probes Editable are the store's after-reading comprehension skills, each its own short passage and question set.
