About this resource

Everything inside: 23 pages · four timed student practice pages (40 facts each — twenty addition, twenty subtraction, shuffled into one grid) · a shared scoring-keys page for all four forms · a data log and graph scored as facts correct per minute · a quick-response observation checklist · a caseload summary · a quick reference card for a second adult · a family update · a 19-starter phrase bank · a binder cover and 4 cut-out divider tabs · a worked, invented example. 392 type-in fields.

"math fact fluency progress monitoring" is a crowded search, and this store already has a timed addition probe and a timed subtraction probe — but neither times the two facts TOGETHER. This file is the interleaved twin: four equivalent forms of forty facts each, twenty addition and twenty subtraction, shuffled into ONE grid instead of two blocks, so a student who is fast inside a single operation still has to prove the switch. Facts within 20 (the same addition/subtraction fact-family identity the two single-operation probes use), answered in writing under a time limit your team sets, scored as speed and accuracy together — facts correct per minute (FCPM) — with a second page that watches HOW the answer came out (recalled instantly, or worked out with a counting strategy) and gives the team a place to write its own call on automaticity, never a cutoff this file imposes.

What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a field, type, Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc, or an Excel workbook, and it is not a norm-referenced test — it carries no fixed fluency benchmark score anywhere in the file. The four student practice pages and the scoring-keys page print; the goal page, log, checklist, caseload page, second-adult reference and organizer are type-in.

It is not the store's existing Addition Facts Fluency probes or Subtraction Facts Fluency probes, which each time ONE operation only — this file interleaves both operations into the same timed grid instead, so the score reflects whether the student can switch between adding and subtracting, not just do one of them fast. All three can be used together across a year: single-operation probes for baseline automaticity, this file once each operation is established, to check that fluency holds up under a mixed task.

It is not a curriculum, and it is not a rule about how many facts per minute counts as "fluent." What fluent enough means for a given student is the team's own judgment, written in from the baseline and the IEP; this file never sets that number on its own — the quick-response checklist page is a blank the team fills in, by design. No age, eligibility or IDEA-timeline claim anywhere; no health data is pre-filled, and the example student (Devon) is invented and named as such on page 22. Facts within 20 only (single-digit addend/minuend pairs 1-9, no zero addend, no borrow-across-tens on the subtraction half) — this first build does not cover multiplication, division or facts outside this range.

Who This Is For

Math interventionists, special education teachers and case managers who need a timed, repeatable way to check whether addition and subtraction fact fluency holds up when the two are mixed together — not just whether each fact is right in its own block — plus a second, condensed page for whoever else gives the probe.

If You Want More

Addition Facts Fluency, Timed Progress Monitoring Probes Editable and Subtraction Facts Fluency, Timed Progress Monitoring Probes Editable are the two single-operation probes this file draws its fact pool from — same goal-log-graph shape, four forms, forty facts a form, scored as FCPM. Math Counting Fluency and Number Identification Speed Probes Editable is the same timed, rate-scored shape for counting and number identification, not fact fluency.
