About this resource
The paper behind an MTSS meeting, in one fillable PDF: a snapshot of the student, what has been tried, the scores and counts over time, goal progress, and a decision form for the meeting itself. Type it, or print it and write by hand. Your school's own rules stay on blank lines in your own words.
EVERYTHING INSIDE — 20 pages
- Student Snapshot: strengths, what worries you in words you could see or count, 4 areas you are tracking
- What We Have Tried: 9 rows for everything tried across the term (what, level, start, end, minutes by days, who, what happened)
- Skill Data Tracker: 15 rows of date, target skill, score, out of, team target and a note
- Score Graph and Roll-Up: a graph for 12 checks on a 10-row scale, with a roll-up (baseline, latest, change, checks done, checks at or above the team target)
- Behavior Tally: two weeks, Monday to Friday by morning, midday and afternoon, with day, block and week totals
- Goal-by-Goal Progress Grid: 6 goals, a baseline and 4 reporting periods
- Tier Meeting Decision Form: data we looked at (4 rows), what we tried (3 rows), 5 decision choices, and a blank line for your school's rule
- After the Meeting: who was at the table (5 rows) and who does what by when (8 rows)
- Classroom Teacher Quick Check: a one-page weekly check for the general education teacher or aide
- Caseload Roll-Up: 15 students on one page, with roll-up counts
- Family Update: blanks for your own wording
- Phrase Bank: 32 observable-language starters in 8 groups
- Two filled examples with an invented student: the data (page 17) and the meeting (page 18), every number re-added from its own table
- A binder cover, a spine label and 6 cut-apart tabs
- Start Here, a contents page with page thumbnails, and a "what's next" back page
How To Use It
1. Start the student file: type the snapshot and what has been tried. Save As once for each student.
2. Collect the numbers: score a skill, count a behavior, track each goal. A classroom teacher can send a week of numbers on the quick check.
3. Hold the meeting: use the decision form, then write who does what by when.
What This Is, And What It Is Not
It is a fillable PDF: the fields are built into the document, so it works offline with no account or subscription. It is NOT a Google Slides, Google Docs or PowerPoint file, and it does not copy numbers between pages, add them up or graph for you. You cannot drag the boxes around or restyle the pages.
What counts as enough data, what a level of support is called, how long it runs, who decides and what happens next are your school's rules. The file prints none of them: targets, decisions and next steps are blank for your words, and the meeting form has a line for your school's own rule. There are no cutoffs, no timelines and no suggested interventions. No health information is filled in. The student and staff in the examples are invented and say so on the page. The cutting is the 6 binder tabs, in one block of straight cuts.
Who This Is For
Special education teachers, case managers, interventionists, school counselors and general education teachers who bring data to a student support or MTSS team meeting, and the families who hear about it.
