- Task Analysis Data Sheet - one routine in up to twelve steps, eight sessions across, the prompt code in every cell (I, V, G, M, PP, FP), and the count and percent of independent steps at the bottom, so the sheet shows which step is holding the routine up.
- Session Data Sheet - seven goals, the prompt used, ten trials, count, percent, notes, and three boxes for materials and positioning, response, and next session.
