What This Is, And What It Is Not

It is a fillable PDF. Open it in a PDF reader, click a line, a box or a cell, type, press Tab to move on, and Save As once per student. It works offline with no account or subscription. It is NOT a Google Sheets file, a Google Doc or an Excel workbook, and it does not sort the entries or send the email.

It is not your district's contact form and not a legal record. Nothing in it says how often to call, how many attempts count, or who may be contacted; each of those has a line for your district's policy, and custody or contact notes are copied from the file as the file states them. It is the working paper you keep so that the district form, the conference and the progress report can be filled in from something written on the day.
