About this resource
Send the first family letter of the year with a questionnaire that is ready to go. One editable PDF holds a one-page parent questionnaire, a quick version for busy families, pages for families to write about their child in their own words, a what-helps page, a home routines page, a hopes and questions page, a how-to-reach-me grid with best times, a filled-in example with an invented family, a class summary for you, and a thank-you reply to send back. Type or print. Every question is optional, so families answer what they want.
WHAT IS INCLUDED - 15 pages, 244 type-in fields
Start Here
- Start Here - three steps and a table that lists every page with its number and what it is for
- Terms of Use - one classroom, how the fillable PDF works, print at 100 percent, and how to keep family answers private
FAMILY PAGES (families type or print)
- Parent Questionnaire - one page: parent or guardian, phone or email, favorite things, what makes my child smile, strengths, school and learning, supports at home, what I want the teacher to know, and contact choices with best times
- Quick Version - 8 questions, about five minutes, with a line for anything else
- About My Child, in My Words - five large boxes with room for several lines each: favorite things, what makes my child smile, what my child is proud of, what I want the teacher to know first, and anything else
- What Helps My Child Learn - 10 things to check (short breaks, reading together, hands-on materials, songs or movement, pictures and visual cues, one step at a time, choices, praise for effort, a quiet place, time to talk), then 4 sentences to finish in the family's own words
- Home and Routines - a table of 6 parts of the day (morning, getting to school, after school, homework time, dinner, bedtime) with "what it looks like" and "what helps" columns, plus who takes my child to school and who helps with homework
- Hopes and Questions - three sentence cards (hopes, looking forward to, a little worried about) and a box with room for your questions
- How to Reach Me - 6 ways to reach the family (email, text, phone call, note home, school app, other), a Monday to Friday best-times grid with four time blocks, and lines for quiet times and adults who may receive notes
What This Is, And What It Is Not
It is a fillable PDF. Open it in a PDF reader, click a line or box, type, and Save As once per family. It works offline with no account or subscription. It is not a Google Sheets file, a Google Doc or a spreadsheet, and it does not add up the best-times grid or sort the class summary for you.
It is not the IEP Parent Input Form and it does not replace any school or district form. It has no health, allergy or medication fields, and it asks for no ages, scores or labels. It is a set of blank pages for a family's own words. Your school decides how family input is used.
Who This Is For
General and special education teachers who send a family questionnaire at the start of the year, case managers who want a plain parent form before a meeting, and paraprofessionals and front-office staff who collect the replies.
