About this resource
The three phoneme-level skills a reading intervention drills together, in one file: isolating a sound in a word, blending separate sounds into a word, and segmenting a word into its separate sounds. Three probe forms of fifteen items each per skill, a shared goal and tracking setup, a response-accuracy tracker for every skill, a sound-family observation page, one glance-page of graphs for all three skills, a caseload summary, a family update and a phrase bank for the progress note.
Every task in this file is oral: the teacher reads from the form and the student answers out loud. Every form is fifteen items and untimed, and no item repeats across a skill's own three forms.

The Three Skills And The Goal
- A one-page map of the three skills, ordered the way most curricula introduce phoneme-level work, with what each skill measures and its page range
- A five-item classroom teacher quick-check for each skill — a fast, ungraphed spot-check a general education teacher can give without starting the full weekly probe routine
- A combined goal and tracking setup page: one row per skill with the sample criterion, a baseline column, this student's criterion, a target date and a met column; shared directions for giving and scoring every probe in the file; a benchmarks table; an "active this marking period" checklist for the three skills
Nine Probe Forms, Three Skills, Three Forms Each (A, B, C)
- Phoneme Isolation: the teacher says a one-syllable word and asks for the first sound or the last sound (eight first-sound and seven last-sound items a form); every word has a single consonant onset and coda, so the target is never a blend or a digraph
- Sound Blending: the teacher says a word broken into separate sounds, the student blends it back into one word; ten three-sound and five two-sound words a form (two-sound words that end in a vowel are said with the long vowel sound, and the goal and tracking page says so)
- Phoneme Segmentation: the teacher says a whole word, the student says every sound in it separately, in order; ten three-sound and five four-sound words a form, no digraphs, so the teacher key is the word spelled with a dash between each letter
- Every form carries the goal it measures: a sample goal in brackets, plus a type-in box for the goal as written for this student
- A teacher scoring strip, 1 to 15, and a Correct ___ / 15 box at the foot of every form
Track, Report And Share
- A response-accuracy tracker for each of the three skills: twelve rows (three baseline days plus nine weekly visits), a rotation reminder, and a notes column
- A sound-family observation page: which consonant sounds are stops, fricatives, nasals, liquids/glides or the one affricate in this file, an optional tally from the isolation tracker's error column, and a blank for the team's own written observation — never a scored or diagnostic claim this file makes for you
- Three small graphs on one page, one per skill, each scaled 0 to 15 over the probe rotation, plus a card on reading the three together
- A caseload summary: one row per student-skill pair, baseline, most recent score, trend and a decision column
- A family update, ready to send home: a checklist of which skills were practiced, a line for something that went well, a five-minute home activity, and the week's scores in plain words (no raw numbers or codes)
- A phrase bank of observable-language starters for a session note, a sound-family pattern, or a quarterly summary line — each starter is built around a count, a pattern or a plan from the tracker, never a label for the student
- A filled-in example: an invented first-grade student, isolation and blending shown with six weekly probes each, the baseline taken as a median, and the progress-report sentence the data produces
What This Is, And What It Is Not
It is a fillable PDF. The goal and tracking page, the three trackers, the sound-family page, the caseload summary and the three-graph page are type-in fields: open the file in a PDF reader, click a line or cell, type, and Save As once per student. The nine probe forms are meant to be printed; every task on them is oral, so the student never reads or writes on the form itself. It works offline with no account or subscription. It does not calculate percents or draw the graphs for you.
It is not a norm-referenced instrument and not a clinical finding. No page sets an eligibility criterion, states a typical age or rate, or names a condition. Every sample goal is invented in brackets; your team sets the real criteria from the baseline and the student's IEP. The sound-family page is a working note for the team, never a scored subtest.
Who This Is For
Reading interventionists, early-literacy and special education teachers writing phoneme-level IEP goals, kindergarten and first-grade intervention and Title teachers, and the paraprofessionals who give the weekly probe.
If You Want A Different Depth Or Breadth
This file trades breadth for depth on just the phoneme-level skills: fifteen items a form instead of ten, on three skills instead of six. Phoneme Segmentation also sells as its own single with six full forms, for a student who needs more practice than the three forms here give. A broader Phonological Awareness file covers six different skills (rhyme, word and syllable segmentation, initial and final sound identification, and sound blending) at three shorter, ten-item forms each; it has no phoneme segmentation, and its initial and final sound identification and sound blending cover the same ground as isolation and blending here, so a team that owns it needs this file mainly for the segmentation forms and the longer fifteen-item forms.
